Prologue · The Eternal Conflict & the Fall of Tristram

Sanctuary • Tristram

Long before a single rune was ever socketed, the angels of the High Heavens and the demons of the Burning Hells waged a war without end over all creation. From that war the mortal world of Sanctuary was born, and with it humanity. When the three Prime Evils were cast down into our world, the Horadrim sealed each brother inside an enchanted soulstone. Centuries later, the youngest of those prisons cracked open beneath a quiet town called Tristram — and the Lord of Terror began to walk again in the body of a man.

  • The Eternal Conflict: Heaven and Hell have fought since the dawn of time, neither side ever able to claim a final victory.
  • Sanctuary, the mortal realm, was created as a refuge by beings who tired of the endless war; its people, descended from both angel and demon, became humanity.
  • Three Prime Evils ruled the Hells — Diablo, the Lord of Terror; Mephisto, the Lord of Hatred; and Baal, the Lord of Destruction. Cast into Sanctuary during the Dark Exile, they sowed corruption across the land.
  • The Horadrim, an order of mages assembled at the urging of the archangel Tyrael, hunted the brothers and bound each within a soulstone. The mage Tal Rasha gave his own body as an eternal prison for Baal.
  • Diablo was sealed beneath the cathedral of Tristram. Generations later his whispers corrupted the archbishop Lazarus and the mad king Leoric, drawing a lone hero down into the labyrinth.
  • The hero struck Diablo down, but to keep the demon contained he drove the cracked soulstone into his own forehead. The corruption grew, and the hero became the Dark Wanderer whose journey east opens the tale of Diablo II.

Diablo · The Lord of Terror (First Defeat)

Beneath Tristram's cathedral the Lord of Terror was beaten back by a single warrior. Victory came at a terrible price: in trying to imprison the demon within himself, the hero became the very vessel Diablo needed to roam free.

Notable foes

  • The Butcher
  • King Leoric, the Skeleton King
  • Archbishop Lazarus

Creatures

  • The Risen Dead: Skeletons and zombies animated by the cathedral's corruption — the first horrors the original hero ever faced.
  • Fallen: Cowardly little imps that swarm in packs and flee the moment their shaman is slain; a recurring nuisance across every act.
  • Hell Spawn: Lesser demons drawn up through the breach Diablo's presence tore in the world.

Townsfolk

  • Deckard Cain: The last of the Horadrim and keeper of Tristram's history. He becomes the player's guide and the one who identifies your hard-won treasures.
  • Tyrael: Archangel of Justice who first warned mortals of the Prime Evils and founded the Horadrim to contain them.
  • Tal Rasha: The Horadrim mage who sacrificed his body to imprison Baal, sealing himself away in a hidden desert tomb.
I · The Sightless Eye

Rogue Encampment • Blood Moor • the Monastery

The Dark Wanderer has passed through the western wilds, and everything he touched has rotted. The Sisterhood of the Sightless Eye, once proud guardians of these lands, have been turned against one another, their dead rising under a corrupted captain. Worse still, the demon queen Andariel has seized the Rogue Monastery that guards the only pass east. To follow the Wanderer, the hero must cleanse the sisterhood's lands and cast the Maiden of Anguish out of her stolen halls.

  • The Wanderer's passage left a wake of corruption; the Rogues who once protected travelers now hunt them.
  • Blood Raven, a celebrated heroine of the Sisterhood, has been twisted into a necromantic horror raising corpses in the Burial Grounds.
  • Deep in the Black Marsh, the greedy Countess hoards a fortune of stolen runes inside the Forgotten Tower.
  • Using the ancient Cairn Stones, the hero reopens a portal to ruined Tristram and rescues Deckard Cain.
  • Andariel has claimed the Monastery to bar the eastern road, serving the Wanderer by sealing the land behind him.
  • With the Maiden of Anguish destroyed, the pass to the deserts of Aranoch is finally open.

Andariel · Maiden of Anguish

A demon lord who delights in suffering, Andariel turned the Sisterhood's own monastery into a charnel house. Her poison clouds and lashing claws make her the first true test of any hero — and her catacomb lair guards the only way east.

Notable foes

  • Blood Raven
  • The Countess
  • The Smith
  • Corpsefire
  • Bishibosh
  • Treehead Woodfist
  • Griswold

  • Carvers & Shamans: A breed of fallen whose shamans resurrect the slain; kill the caster first or the fight never ends.
  • Zombies & Ghouls: Slow, relentless dead that shamble out of the cold plains and burial grounds.
  • Hungry Dead / Returned: Skeletal warriors haunting the Crypt and Mausoleum beneath the Burial Grounds.

Townsfolk

  • Kashya: Captain of the Rogues; once trust is earned she allows you to hire Rogue Scouts as mercenaries.
  • Charsi: The encampment blacksmith, eager to recover her enchanted Horadric Malus from the Monastery forge.
  • Warriv: Caravan master who eventually carries the hero across the desert to Lut Gholein.
  • Gheed: A traveling merchant with a sharp eye for profit and a rotating stock of gear and gambles.
II · The Secret of the Vizjerei

Lut Gholein • the Tombs of Tal Rasha

The trail leads to Lut Gholein, a sun-baked port on the edge of the Aranoch desert. Here the Dark Wanderer searches for the hidden tomb of Tal Rasha, where Baal has lain imprisoned for ages. To reach the true tomb among seven decoys, the hero must rebuild the long-lost Horadric Staff and brave the shifting maze of the Arcane Sanctuary. But the Wanderer is always a step ahead — and what waits in the tomb is not rescue, but the Lord of Pain.

  • The Wanderer seeks Tal Rasha's tomb to release Baal, the Lord of Destruction, from his ancient prison.
  • Radament, a Horadrim mummified and driven mad in the sewers beneath the city, guards forgotten knowledge.
  • The Horadric Staff must be reforged in the Cube from the Staff of Kings and the Amulet of the Viper.
  • The Summoner, a corrupted mage, holds the Arcane Sanctuary — the mirrored dream-realm of the sorcerer Horazon.
  • Among the seven false tombs lies the true resting place, sealed and watched by Duriel.
  • The hero arrives to find Tyrael chained and Baal already freed; the archangel sends the hunt onward to Kurast.

Duriel · Lord of Pain

Brother to Andariel and jailer of the true tomb, Duriel is a hulking maggot-demon who chills the air and crushes the unwary in a cramped chamber with nowhere to run. He is the desert's final, brutal gatekeeper.

  • Vultures & Sand Leapers: Swift desert predators that close distance fast and harry travelers on the open sands.
  • Burning Souls: Floating lantern-spirits that hurl deadly bolts of cold fire from afar in the deeper tombs.

Townsfolk

  • Atma: A grieving tavern keeper who lost her family to the sewer plague and aids the hero in return for vengeance.
  • Drognan: A learned desert mage who sells spells and shares hard truths about the Wanderer's purpose.

  • Greiz: Captain of the local mercenaries; through him the hero can hire Desert Mercenaries (Iron Wolves).
  • Meshif: A sea captain whose ship eventually carries the hero across the waters to the jungles of Kurast.
III · The Infernal Gate

Kurast Docks • the Jungles • Travincal

Across the sea lies Kurast, a once-glorious city now strangling in jungle rot. Mephisto, the Lord of Hatred, has poisoned the Zakarum religion at its root, twisting its high council into his devoted jailers. To breach the temple where he is bound, the hero must reclaim the scattered remains of the martyr Khalim and forge his Will — the only weapon that can shatter the orb sealing the way down. Beyond the Durance of Hate, the Infernal Gate itself stands open.

  • Mephisto has corrupted the Zakarum faith, turning its zealots and high priests into servants of Hatred.
  • The High Council of Travincal guards Mephisto's soulstone at the heart of the temple city.
  • Khalim, the last untainted councilman, was murdered and dismembered; his eye, brain, heart, and flail lie hidden across the jungle.
  • Combined in the Cube, Khalim's Will can destroy the Compelling Orb that bars the Durance of Hate.
  • Mephisto is confronted and slain in the depths beneath the temple, his soulstone claimed.
  • His defeat tears open the Infernal Gate, through which the Dark Wanderer steps into the Burning Hells.

Mephisto · Lord of Hatred

Eldest of the three brothers, Mephisto rules through corruption rather than force, rotting hearts and institutions from within. Imprisoned in the Durance of Hate, he wields skull missiles, charged bolts, and chilling power — a fight best won across the safety of a moat.

Notable foes

  • Toorc Icefist
  • Ismail Vilehand
  • Geleb Flamefinger
  • Bremm Sparkfist
  • Wyand of Voidbringer

Townsfolk

  • Ormus: A haunted mage who senses Mephisto's grip on the city and sells spells and healing.
  • Hratli: A sorcerer-smith stranded on the docks; he repairs and sells gear and craves escape from the jungle.
  • Asheara: Leader of the Iron Wolves; through her the hero can hire skilled elemental mercenaries.
  • Alkor: A reclusive alchemist whose potions and rare stock reward those who do him a quiet favor.
  • Natalya: A mysterious assassin who watches the council and hints at a greater threat gathering in the far north.
  • Deckard Cain: Ever the loremaster, Cain follows the hunt and continues to identify and counsel along the way.
IV · The Harrowing

the Pandemonium Fortress • the Chaos Sanctuary

The chase ends in the Burning Hells, where the last angelic bastion — the Pandemonium Fortress — clings to the border of damnation. Here the hero learns the bitter truth from a fallen angel's freed spirit, shatters a brother-demon's soulstone in the Hellforge, and breaks the seals of the Chaos Sanctuary to draw out Diablo at last. Yet even in death, the Lord of Terror has only been buying time for the brother who already escaped.

  • The Pandemonium Fortress is the final foothold of Heaven's forces on the edge of the Burning Hells.
  • Izual, once a proud lieutenant of Tyrael, betrayed Heaven's secrets to the Hells; his tormented spirit is set free at last.
  • At the Hellforge, the hero shatters Mephisto's soulstone with the Hellforge Hammer, denying the demon any return.
  • Three Infernal seals are broken in the Chaos Sanctuary, releasing their guardians to summon Diablo.
  • Diablo, the Lord of Terror, is finally destroyed in open battle.
  • The victory is hollow: the Dark Wanderer's true purpose was a diversion, and Baal has already marched north toward Mount Arreat.

Diablo · Lord of Terror

Freed from his mortal vessel and restored to full demonic glory, Diablo is a towering nightmare of fire and lightning. His infamous Lightning Inferno can kill an unprepared hero in an instant — survive it, and the Lord of Terror falls in the Chaos Sanctuary.

  • Jamella: An angelic quartermaster who sells gear and gambles within the fortress walls.
  • Halbu: The fortress smith who repairs arms and armor for the war against the Hells.
  • Deckard Cain: Still at the hero's side, Cain identifies the spoils torn from Hell itself.
V · Lord of Destruction

Harrogath • the Slopes of Mount Arreat

Baal has reached the frozen north, laying siege to Harrogath — the last barbarian stronghold guarding the slopes of Mount Arreat. Within the mountain rests the Worldstone, the ancient artifact that anchors all of Sanctuary and hides it from Heaven and Hell alike. Betrayed from within and beset by Baal's army, the defenders' only hope is the hero: to lift a curse, pass the trial of the Ancients, and chase the Lord of Destruction into the Worldstone Keep before he can corrupt it forever.

  • Baal's demonic army besieges Harrogath, butchering the barbarians who have guarded Mount Arreat for generations.
  • Nihlathak, an elder of the city, betrayed Harrogath and stole the Relic of the Ancients in a doomed bid to spare it.
  • Anya, a young alchemist, is captured and frozen by Nihlathak's treachery and must be rescued and freed.
  • The Ancients — three spirit-guardians of the barbarian people — bar the summit and must be bested in single combat.
  • Through the Worldstone Keep the hero pursues Baal, who corrupts the Worldstone even as he is finally slain.
  • To keep the tainted Worldstone from the Hells, Tyrael shatters it — and Mount Arreat is sundered in the blast.

Baal · Lord of Destruction

Youngest and most cunning of the Prime Evils, Baal commands waves of demonic minions before facing the hero himself in the Worldstone Chamber. He clones himself, drains mana, and hurls devastating cold and decay — the final boss of the original game and its expansion.

Townsfolk

  • Larzuk: The Harrogath smith whose unique gift is adding sockets to your gear — invaluable for runewords.

  • Anya: Once rescued, she personalizes resistances, sells gear, and opens the way toward Nihlathak's lair.
  • Qual-Kehk: Commander of Harrogath's defense; he resurrects fallen mercenaries and lets you re-hire barbarian warriors.
  • Tyrael: The archangel makes his final, fateful choice here — destroying the Worldstone to save Sanctuary.
